Registration for our Fall 2012 season will  take place at Kings Plaza Shopping Center
located on Flatbush Avenue @ Avenue U - Brooklyn
Saturday, April 21  from 12p -4p
Sunday , April 22  from 3p -5p
Saturday June 9 from 12p -4p
Registration Fee (Includes all games, practices, uniform, trophy, secondary insurance):
     $90. per player with pre-printed registration forms 
     $115. per player without pre-printed registration forms 
2012 Fall Season begins Sunday, September 9th and runs til November 18th


STEP ONE

The first step is to register at eAYSO , where you input your own personal information directly into the American Youth Soccer Organizations database.  Upon completion, you'll print and bring copies of your completed forms to our next registration event  where we will check them for accuracy, verify your documentation, and accept payment (CASH, CHECKS, MONEY ORDERS - no credit cards please).  If you come to our registration event without these completed printed forms, a $25. processing fee will be applied to each new and returning players registration.  

NEW PLAYERS -  must register a parent as a VOLUNTEER and the child as a PLAYER at eAYSO 
RETURNING PLAYERS - simply check information at eAYSO for accuracy.
VIP (Very Important Players) - need only register the child as a PLAYER and check the VIP box -  A parent may register as a VOLUNTEER if they so choose, however it is NOT mandatory in the VIP division.

STEP TWO  (you will not be registered unless you follow this step)
Print one copy of the players form (one for each player registering) and one copy of the volunteer form, and bring to our registration event, where we will check the forms for accuracy, verify your documentation, and accept payment (CASH, CHECKS, MONEY ORDERS - no credit cards please).  

WHEN YOU ARRIVE AT REGISTRATION PLEASE HAVE:

One copy of the player form (one for each player registering) and one copy of the volunteer form.
Players birth certificate (NEW PLAYERS ONLY)
State photo ID (ALL PARENTS/GUARDIANS)
Volunteer's Social Security number (NEW VOLUNTEERING PARENTS)
Means of payment (CASH, CHECKS, MONEY ORDERS - no credit cards please)

PLEASE NOTE THE FOLLOWING

  • Filling out these forms online at eAYSO ONLY registers you with the national organization.  You are NOT fully registered until you bring these forms in person to our (Marine Park Soccer/AYSO Region 266) registration event, along with additional documentation (ex.  ID, birth certificate, SS#, etc.) and payment.
  • Coming to a registration event without printed completed registration forms or with hand written forms, whether because of printing problems caused by eAYSO or your printer will result in the addition of a $25. processing fee for a total registration fee of $115. for each new and returning player registration.  If you experience trouble printing your forms, please click the "HELP" button on the eAYSO website.   eAYSO is run by the AYSO national office in California, which we have no control over.
  •  You are joining an ALL VOLUNTEER organization.  One parent/guardian (the volunteer) will be given something to do for the season.  Extenuating circumstances resulting in an inability to volunteer must be approved by the board before the season starts.
  • Background checks will be performed on all volunteers.
